|
Death Certificates are issued for those deaths that occur in Basking Ridge, Liberty Corner, and Lyons only.

Checks are to be made payable to:
"Bernards Township".

Only a direct family member, i.e. spouse, sibling, or child can request a certified copy.
Proof of relationship must be accompany the request. For example, a child would need to provide a full copy of
their birth certificate showing their parents’ full names on the certificate

Identification must accompany a request for a death certificate such as a valid photo driver's license.
|
Please print and complete the certificate request and mail with a stamped self-addressed return envelope
to the Bernards Township Health Department at the address below.
