BIRTH CERTIFICATE
Birth Certificates are issued for those births
that occurred in Basking Ridge, Bernards
Township only. The fee
for a birth certificate is $10.00. Identification must accompany a request for
a birth certificate and the certificate can only be issued to a parent or the
individual himself. Please print and complete the certificate
request on this website and mail to the Health Dept.
with a stamped self-addressed return envelope. Checks are to be made payable to
Bernards Township.
DEATH CERTIFICATE
Death Certificates are issued for those whose death occurred in Basking
Ridge, Bernards Township only. The fee for a death
certificate is $10.00. Identification must accompany a request for a death
certificate. Only a direct
family member, i.e. spouse, sibling, or child can request a certified
copy. Proof of relationship must be
accompany the request. For example,
a child would need to provide a full copy of their birth certificate showings
their parents full names on the certificate. Please print and complete the certificate request on this website and
mail to the Health Dept. with a stamped self-addressed return envelope. Checks
are to be made payable to Bernards
Township.
MARRIAGE CERTIFICATE
Marriage Certificates are issued for those whose marriage occurred in
Basking Ridge, Bernards
Township only. The fee
for a marriage certificate is $10.00. Identification must accompany a request
for a marriage certificate. Please print and complete the certificate request on this website and
mail to the Health Dept. with a stamped self addressed return envelope. Checks
are to be made payable to Bernards
Township.
MARRIAGE LICENSE
Either the bride or the groom must be a resident of the municipality of
Basking Ridge, Bernards
Township. This license is good for use anywhere in
New Jersey.
If both the bride and groom are non-residents of New Jersey, the license must be obtained
from the Registrar of the municipality where the marriage is to be performed
and is only good for use in that municipality.
The license fee is $28.00. The office hours for application of Marriage
license are Monday through Thursday 9 a.m. to 4 p.m. and Friday 9 a.m. to 3
p.m. Appointments must be made in
advance. A witness 18 years or older must accompany the couple at the time of
the application. The registrar will not issue a marriage license sooner than 72
hours after the application for a marriage license has been made, unless
ordered to do so via an appropriate court order. The marriage application MUST
be completed by both parties before the license will be issued. Once the marriage
license is issued, it is good for 30 days from the date of issuance.
A certified copy of a birth certificate, or valid passport will be
requested. If one or both of the applicants are divorced or widowed, you must
bring a copy of the final decree(s) or certified death certificate(s) for
examination by the Registrar.
